Submitting this form serves as an electronic signature and agreement to the following: I understand that Rishone Todd, Susie Kushner and other faculty of Musicality Central are well-trained qualified professionals who teach safe dance. Musicality Central operates under an assumed-risk clause, and I will under no circumstances hold any instructors of Musicality Central or studio locations in which classes are taken liable for any injury or ailment I may obtain due to my negligence or folly during visits at studios in which Musicality Central teaches. I understand and agree that in participating in any dance class or workshop, there is a possibility of physical injury or death. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me during any Musicality Central classes. I also exempt, release, and indemnify Musicality Central instructors and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, or property which may arise out of or in connection with participation in any classes or activities conducted by Musicality Central. Permission is granted for Musicality Central to use photographs of students for publicity purposes.
